Failure to provide this information means that we will not be able to process your enrolment application and you will not be accepted onto our teacher training course.

Classes and Workshops

For our class and workshops, we collect the following types of information about you: Your name, address, contact telephone number and email address. We collect this data in order to communicate with our clients to send class and workshop confirmations and reminders, class pass purchases and renewals, refunds and cancellations. MEDICAL SCREENINGS

In order to safeguard the health of our clients, we ask clients to complete a medical screening questionnaire on their first visit to Yoga Mama. The information gathered on this form is strictly confidential and is solely to inform our teachers of any medical conditions. This information enables our teachers to provide safe alternative options during classes and workshops, and, where necessary, advise clients from undertaking the class/ workshop that they wish to sign up for. We scan these paper files and add them to The Putney Clinic of Physical Therapy’s cloud-based online filing system. Once added, we securely shred the paper files.

New online clients are requested to fill in an online medical screening when booking their first class. As with paper questionnaires, the information collected enables our teachers to provide safe alternative options during classes and workshops. Where necessary, this also allows teachers to advise clients from undertaking the class/ workshop that they wish to sign up for.

WEBSITE APPLICATION FORMS

Should you wish to apply to enrol on one of our teacher training courses, you can use the online application forms on our website. We request that you complete all fields, so that we can process your application. All messages sent via the application form are sent to our course administrator on christine@yogamama.co.uk.
